Separated log streams for access log (defaults to stdout) and general
log (defaults to stderr). Added support for writing logs to a file.

Workaround for missing parameters in GPN response

Some clients don't report certain parameters in GPN response but report
their descendants only. This patch attemps to add those missing
parameters assuming writable attribute being false.

New and improved fault management
...
This introduces the concept of "channel". Presets may belong to different
channels such that if one preset causes a fault, only presets belonging
to the same channel will be paused. Presets in other channels will continue
to be applied.
